PASSPORT SERVICES
The Lake County Title Bureau is proud to serve as a passport acceptance office for the people of
Lake County. Please see the following information regarding what to bring with you to your
appointment, how much the passport will cost, and how long the process will take from your
visit to the Title Bureau to when you receive your passport!

Passport Appointments:
Appointments are recommended to ensure timely processing for your Passport. The Title Bureau processes Passport/Passport Photos By Appointment on the following dates and times:

The U. S. Secretary of State has designated various categories of persons who are authorized to
accept passport applications. Pursuant to that authority, the Clerk of Courts in Lake County is
accepting passport applications as a service to the public at both Title Bureau Offices. For more

A passport application must be accompanied by four items:

Proof of U.S. citizenship

Proof of identity
-

Proof of U.S. citizenship

Submit a previous U.S. passport or a certified copy of a birth certificate. Children under 16 must
have both (if a previous passport was issued). If no birth certificate was issued, a letter of “no
record” and secondary evidence may be submitted for consideration. -

Proof of identity: Submit one of the following, with a copy of front and back

A valid previous U.S. or foreign Passport

A valid driver’s license

A valid Military I.D. card

A valid State I.D. card

Photographs
One photograph is required which must have been taken within the last six months. It must be in color and be exactly 2”x 2”. The photograph must be whole face view without any part of head or hair cut off. No glasses or hats may be worn. Photo must have a white or off-white background and be facing forward. (Vending machine photos are not acceptable.)
Where can I get a passport photo?
We provide passport photo services at both Title Bureaus at the cost of $15.00 each.

Passport Book Fees:
Normal Processing (Must have check or money order, payable to U.S. Dept. of State)
• Adult (age 16 and over) $130.00 for 10-yr. passport
• Child (age 15 and under) $100.00 for 5-yr passport

Expedited Processing (Must have check or money order, payable to U.S. Dept. of State)
• Adult (age 16 and over) $160.00 for 10-yr. passport
• Child (age 15 and under) $190.00 for 5-yr passport
NOTES: A processing fee of $35.00 per person will be collected, payable by credit card, cash, check or
money order (payable to Faith Andrews, Clerk of Courts). Starter checks are not accepted.
The person whose name appears on the card or check must be present.
